This fund shall provide ongoing in-home needs assessment, care planning, and consultation to help vulnerable seniors maintain their independence through supportive services of assistance in financial management, medical appointments, insurance, transportation, housing needs, essential food needs and medical care including financial aid for any of these needs and services when necessary.
