Pathways Endowment Fund
Purpose
Pathways’ mission is to provide compassionate care to families living with illness and loss. Through a suite of programs tailored to meet the needs of individuals ages 5 – 65+, we deliver vital services and support to the Greater Long Beach community at no cost – eliminating barriers in access to care to clients and their families. Our objectives are to provide: 1) case management, volunteer companionship, and home supportive services to frail/elderly and chronically ill individuals through our Care Navigation Program 2) quality of life, dignity, and compassionate care to terminally ill patients and their families through our Hospice Program, and 3) grief counseling, education, crisis response and bereavement support to children through our school-based CHANGES Program, and adults and seniors through our community-based CHANGES and Journeys Programs. Our goal is to continue to provide free, high quality, accessible services that meet local needs.
